Hi, I'm Sarah.
Licensed Occupational Therapist · Certified Aging in Place Specialist · Discharge Specialist · 9 Years Experience · MUSC Graduate
With my experience in acute care and inpatient rehabilitation, I help individuals and families navigate the transition from hospital to home and the challenges that often follow. I’m trained in home modifications that support aging in place and help people adapt to traumatic injuries and progressive diagnoses.
In addition to my previous experience, I work in memory care, where I assess residents for environmental barriers and create personalized plans—including home modifications, daily routines, and exercise programs—to promote independence and improve quality of life for those living with dementia and Alzheimer’s disease.
My goal is to be a virtual guide you can turn to at anytime, a supportive and knowledgeable resource in your back pocket as you navigate caregiving at home.
